Description: RGB Group, Inc. is in need of Registered Nurse - Case Manager - Camp Pendleton right now, this vacancy will be placed in California. More details about this vacancy opportunity kindly see the descriptions. The duties for this Full-Time position include but are not limited to the following:

Participate in all phases of the Case Management Program (CMP) a! nd ensure that the CMP meets established case management (CM) standards of care. Assist in coordinating a multidisciplinary team to meet the health care needs, including medical and/or psychosocial management, of specified patients.

Serve as consultant to all disciplines regarding CM issues. Provide nursing expertise about the CM process, including assessment, planning, implementation, coordination, and monitoring. Identify opportunities for CM and identify and integrate local CM processes.

Develop and implement local strategies using inpatient, outpatient, onsite and telephonic CM; develop and implement policies and protocols for home health assessments and outcome measures.

Develop and implement tools to support case management, such as those used for patient identification and patient assessment, clinical practice guidelines, algorithms, CM software, databases for community resources, etc.

Integrate CM and utilization management (UM) a! nd integrate nursing case management with social work case man! agement. Prepare routine reports and conduct analyses.

Assist in establishing and maintaining liaison with appropriate community agencies and organizations, the TRICARE Lead Agent office, and the Managed Care Support Contractor. Qualifications Possess education as follows: Either (a), a baccalaureate degree in Nursing; or (b) a diploma in Nursing; or (c) an associate degree in Nursing from an accredited university.

Possess and maintain a current, unrestricted license to practice as a registered nurse in any one of the 50 States, the District of Columbia, and the Commonwealth of Puerto Rico, Guam or the U.S. Virgin Islands.

Possess and maintain one of the following certifications:
Certified Case Manager (CCM) issued by the Commission for Case Manager Certification, Certified Disability Management Specialist (CDMS) issued by the Certification of Disability Management Specialists Commission, Certified Rehabilitation Registered Nurse! (CRRN) issued by the Association of Rehabilitation Nurses, Certified Occupational Health Nurse (COHN) issued by the American Board for Occupational Health Nurses, Advanced Certification in Continuity of Care (ACCC) issued by the National Board for Certification in Continuity of Care, Certified Rehabilitation Counselor (CRC) issued by the Commission on Rehabilitation Counselor Certification, Nurse Case Manager (RN-NCM) issued by the American Nurses Credentialing Center, Care Manager Certified (CMC) issued by the National Academy of Certified Care Managers.

Possess a Bachelor of Science in Nursing degree.

OR

Possess a minimum of 12 months of case management experience, and show evidence of active pursuit of certification. The HCW shall obtain any of the certifications above within 24 months of contract start.

OR

Possess a Masters degree in Nurse Case Management and obtain any of the certifications above within 24 months of contrac! t start.

Possess a valid drivers license and a personal vehicl! e for use under the contract.

Possess the necessary knowledge, skills, and computer literacy to interpret and apply medical care criteria such as InterQual.
